Our partners have been working together to achieve the goals of the Community Health Improvement Plan (CHIP). This is a demonstration of how working together as a Collaborative, we have a greater impact on our communities.

This is a demonstration of the collaboration of multiple organizations sharing a common goal and implementing services, programs and interventions that may ultimately improve health outcomes. Organizations that provided outreach and educational materials are also listed below.

Individuals Impacted: 150
70 seniors attend senior program, 20 were assisted with navigating healthcare, 200 attended substance abuse community wide event, electronics health board viewed by 500, 6 had cooking companions, 6 received caregiver support group, 2 participated in winds early memory loss group , 150 received case management services ( assistance with obtaining health insurance, navigating health system, following up with doctor recommendations)

Atlantic Health

Presentations were conducted at Chilton – CAB, Middle Manager’s Meeting, various long term care facilities, and Chaplains council and Launch event. Individuals reached include 110 professionals (hospital staff, LTC staff), 77 community members – religious organizations, Senior Center residents, 9 Individuals – Butler Senior Community, 15 individuals – Shomrei Torah, 12 healthcare staff – Windsor Healthcare, and 25 Individuals – NJHC/PCHC.
