Our partners have been working together to achieve the goals of the Community Health Improvement Plan (CHIP). This is a demonstration of how working together as a Collaborative, we have a greater impact on our communities.

Number of Individuals Impacted are added to the progress tracker because they participated in a screening, received a referral or were provided access to care or services, showed positive changes or demonstrated improved knowledge related to the referenced indicator. This is a demonstration of the collaboration of multiple organizations  sharing a common goal and implementing services, programs and interventions that may ultimately  improve health outcomes. Organizations that provided outreach and educational materials are also listed below.

Aging in Place (AIP) programs helping low-income seniors with modifications and safety assessments: 79 Individuals Impacted
2020:
Aging in Place (AIP) programs helping low-income seniors with modifications and safety assessments: 52 Individuals Impacted
24-week exercise and education program for older women and men at risk for, or who have osteoporosis. The program is offered at community sites throughout the state: 324 Individuals Impacted
Complimentary Seminars, elder law and medicaid planning: 300 individuals (Outreach)
Jefferson Seniors Hypertension Screenings: 152 Individuals Impacted
Senior fitness and wellness programs/activities participation: 792  Individuals Impacted
Conducted 8 education programs for older adults in 2019 in Morris County attended by a total of 165 people. (Outreach)
 
Chronic Disease Self Management and Chronic Pain Management Programs. 19 Individuals Impacted
